About: Overview of Project

The Community Chronic Care Network (CCCN) built on a history of productive collaboration among public, private, and not-for-profit healthcare organizations in Santa Cruz County to ensure that all people with chronic conditions receive consistent and continually improving care. The strategies for achieving this vision included employing electronic tools and sharing clinical practice innovations among diverse providers of care. From October 2004 to September 2007, CCCN received grant funding from the Agency for Healthcare Research and Quality (AHRQ) to implement a community-wide electronic registry for diabetes care. This website details the work of CCCN to build the collaboration, clinical consensus, legal agreements and technology for this project. The registry was discontinued in September 2007.
