Overview

The Diabetes Registry Program is a web software application that allows clinicians to view past patient vitals, lab, and pharmacy data for individual patients with diabetes while the patient is present in the office. Clinicians will use the patient Diabetes Visit Worksheet that contains all relevant information to the patient the in office during the encounter. Medical office staff will maintain the patient's diabetes data by accessing the patient's Diabetes Visit Information online, updating office visit information and using the information to contact patients upon a clinician's direction.

A semi-dynamic prototype of the Diabetes Registry Program is available.

Diabetes Registry Program Site Map

The Diabetes Registry program provides access to all possible user functions available to the user. The Diabetes Registry program allows a user to:

  • View a patient's past visit record
  • Record new visit data into the registry database through the online visit form
  • Print one or more visit forms for scheduled patient-doctor visits
  • Add a new patient to the registry
  • Request a patient be deleted from the registry
  • Contact support to get technical assistance

The site navigation organizes access around the main user tasks.

Log in

The Log in page is the entry point into the Diabetes Registry for all users having valid login accounts to use the program. All transactions begin from this page.

View Patient Record - Level 1

View Patient Record page is the first main navigation a user sees when the Diabetes Registry program is initially opened (also see View Patient Records Wire frame). This is where the Diabetes Registry user will spend most of their time either printing out patient visit forms (specific data and blank) and/or adding patient visit information from the last clinician-patient encounter.

Access

View Patient Record Lookup (level 1) is accessed by : (1) log in, then View Patient Record is the default page, (2) click the View Patient Record tab from any other tab pages in the Diabetes Registry program.

Diabetes Visit Record (View Patient Record Level 2)

The Patient's diabetes visit information can be viewed and/or changed by selecting a found patient name from the patient-lookup function and clicking "Display patient record." (also see Diabetes Visit Information wire frame)

Access

Diabetes Visit Information is accessed by : (1) log in /View Patient Record (default page) / click Display patient record button, (2) clicking the tab View Patient Record from any other tab pages in the Diabetes Registry program/ click Display patient record button (3) clicking the tab View Patient Record from any other tab pages in the Diabetes Registry program/ click patient name link in list, (4) clicking the tab Print Worksheet(s) tab from any other tab pages in the Diabetes Registry program/ click patient name link in list, (5) clicking the tab View Patient Report tab from any other tab pages in the Diabetes Registry program/ click patient name link in list,


User Interaction Summary

There are 3 button actions for the user to choose on the CCCN Patient Web Visit Form:

1. Save: User clicks Save and the changes entered on the visit form are saved to the database and the page remain displayed for further browsing and editing.

2. Print worksheet: User clicks Print worksheet and the changes entered on the visit form are saved to the database, a request to print the Patient Visit Form is sent to the printer (print PDF worksheet) and the page remain displayed for further browsing and editing.

3. Exit: User clicks Exit and 2 cases have been identified:

a. User entered changes that have not yet been saved to the database. A dialog box is displayed querying the user about discarding the changes (i.e., Discard button), saving the changes (i.e., Save button), or canceling the Exit action (Cancel button)..

b. User has not entered any changes onto the form before clicking Exit. The Diabetes Visit Record page closes and the page updates with the patient list from the last lookup.

When the user releases the mouse button on a chosen section subheading, the page automatically scrolls to that chosen section location on the page.

Save Changes Before Closing Dialog

The Save Changes Before Closing page appears when the user clicks Exit on the Diabetes Patient Record page or the Add Patient page, level 2 and changes that have been entered have not been committed to the database. This intermediate dialog page prompts the user to either discard changes made prior to clicking Exit, cancel the exit action and go back to editing the patient record information, or save the changes that have not be committed to the database before closing that edit session.

TheSave Changes Before Closing page is accessed by: 1 ) View Patient Record/ search patient name by Name and Birthdate / click Look up/ select patient's name (link clicked)/ enter patient information / click Exit button, or 2) Add Patient / enter patient name by Name and Birthdate / click Add/ enter patient information/ click Exit button.

CCCN Patient Visit Worksheet

The CCCN Patient Visit Worksheet is a PDF document that contains printer formatting information to allow for a nice printed version of the patient visit worksheet that clinicians can use during one-on-one patient encounters. This is the paper version of the online Patient Visit Form.

The Diabetes Visit Worksheet is accessed after the user logs on to the Diabetes Registry program from a number of access points: (1) View Patient Record /Select Specific Patient (results from lookup), (2) Print Worksheets/Select Specific Patient (results from lookup), and (3) Add Patient/Add Patient Demographics. CCCN Patient Visit Blank Worksheet

The Blank Diabetes Visit Worksheet is a PDF document that contains printer formatting information to provide a nice printed version of the patient visit worksheet that clinicians can use during one-on-one patient encounters. This is the paper version of the online Patient Visit Form.

The Blank Diabetes Visit Worksheet is accessed after the user logs on to the Diabetes Registry program from (1) View Patient Record /Select Specific Patient (results from lookup), and (2) Print Worksheets. This form differs from the patient specific Diabetes Worksheet in that new data has not yet been recorded, hence the form contains the fields pertaining to the upcoming visit with no historical data. This form is also only 2 pages (vs. the 3 pages in the patient specific case). (May also download Blank Diabetes Worksheet PDF file.

Access

The blank Patient View Worksheet is accessed by: (1) Log in /Print Worksheets tab/ click (3) click the Print blank worksheet link. A blank worksheet can also be accessed from the View Patient Record page.

Attestation Page

The Attestation page appears when a provider accesses a patient's name through the patient look-up bar but is not assigned as that patient's primary care provider (also see CCCN Site Map showing where in the transaction sequence this page appears).

Access

The Attestation page is accessed by: View Patient Record/ search patient name by Name and Birthdate / click Look up/ select patient's name (row is highlighted) / click Display Patient Record button.

Add Patient (Level 1)

The Add Patient page allows a user to add a patient to the Diabetes Registry. Access to the function will likely be granted to a system admin super user. If the user does not have access to this function, the Add Patient will not appear as a selection in that user's browser. (also see Add Patient wire frame - level 1) The Add patient function first displays the Look up bar to request the user to input the name of the new patient. The rest of the page is blank. When the new patient is entered and the Add patient button is clicked, the system checks the data entry against the database to ensure that this patient does not exist.

Add Patient page (level 1) is accessed by : (1) log in, (2) clicking the Add Patient tab from any other page in the Diabetes Registry program.

 

 


 

Add Patient (Level 2)


Overview

The CCCN Add Patient )level2) buttons have been revised to streamline the user interaction, reduce user's choices and promote user understanding.


Access Point

The CCCN Add Patient is accessed from Login/Add Patient/(enter unique patient name) click Add


User Interaction Summary

Remove Patient (Level 1)

Remove Patient page provides the mechanism to request the removal of a patient from the registry. This "request"is sent to the CCCN technical support team where the actual removal of the patient occurs. (also see Remove Patient Level 1 wire frame) This page functions like the View Patient Record and Print Worksheets pages in that the user must first "look up" the patient(s) to remove before a removal request can be made.

Confirm Remove From Diabetes Registry (Level 2 of Remove Patient)

The Confirm Remove From Diabetes Registry page (level 2) allows a clinician or their staff to specify why the selected patient needs to be removed from the Diabetes Registry and confirm the removal action. The user is presented the following choices of why the patient should be removed from the database: (1) Clinician states patient does not have diabetes, (2) Patient is deceased, or (3) Patient moved out of the area, (4) Informed refused to participate. (also see Remove Patient Level 2 wire frame) When the reason for removing the patient from the registry is specified, an online email notification is sent to the CCCN technical support team for actual removal from the Diabetes Registry database.

Confirm Remove From Diabetes Registry (level 2) is accessed by: Log in/ click the Remove Patient (level1) tab from any other page in the Diabetes Registry program/ search for patient using the Look up bar/ click the Remove from registry button.

View Patient Reports

View Patient Reports page allows the user to obtain lists of patients by physician who exceed or meet specific criterion: (1) Last visit greater than a year ago, (2) Last retinal test greater than a year ago. (3) Last HbA1c test greater than a year ago, (4) Last HbA1c test greater than 9, and (5) Last HbA1c greater than 8 and increased by 1 in previous years. (also see View Patient Reports wire frame)

View Patient Reports is accessed by : (1) log in, and (2) clicking the View Patient Reports tab.

Contact Support

Contact Support page provides an online email request to contact a CCCN technical representative should a user have trouble with information in the Diabetes Registry database or has difficulty using the program interface. A phone number is also supplied in case users wish to call for assistance directly. (also see the Contact Support wire frame)
