The documents have been classified into one of four project areas—collaboration; clinical; legal and information technology. Links to many of these documents are also available in the History section of this website.
• Collaboration refers to work building and sustaining the collaboration of partners to support a community-wide diabetes registry. Most of these documents represent the work of the CCCN steering committee.
• Clinical refers to any work activity relating to the point of care registry tool including the work of the CCCN Clinical committee. The clinical archives are divided into adoption, point-of-care and training documents.
• Legal refers to the legal documents developed for this project.
• Information Technology refers to the work products of the design, development and maintenance of the registry software including the point-of care interface. Documentation of the user interface is also included in the Registry section of this
