Vision

The Community Chronic Care Network will ensure that all people with diabetes in Santa Cruz County receive consistent and continually improving medical support by employing modern electronic tools and sharing clinical practice innovations among our diverse providers of care.

 

Background

The Community Chronic Care Network (CCCN ) is a partnership of Santa Cruz County health care providers to improve communitywide diabetes care via an interactive, web-based diabetes registry. The Network is funded by a grant from the Agency for Healthcare Research and Quality (AHRQ) for a 3-year period starting September 30, 2004. The Pajaro Valley Community Health Trust (Trust) is the recipient of the grant funds and is responsible for financial stewardship of the Network. Health Improvement Partnership of Santa Cruz County (HIP) is the legal entity for Registry implementation. Participating agencies and providers are contributing matching services and supplies to the Network.

 

Purpose

The purpose of the Clinical Committee is to oversee the clinical features of the electronic chronic care system to support and champion adoption throughout the provider community. This includes ensuring the embedded clinical guidelines are updated; sharing of best practices; serving as an avenue for provider input into the registry, and defining and analyzing outcomes data.


The Committee recommends strategies and priorities for improving communitywide clinical outcomes for persons with diabetes and other chronic conditions to continue improvement in chronic care beyond the grant period. The Committee will also provide the clinician’s perspective in the development of a community-wide chronic disease registry.

 

Membership

The Clinical Committee includes representative of diabetes care providers in Santa Cruz County including primary and specialty care and diabetes self management education. The organizations and their representatives for 2006-2007 are:

Meeting Structure

The Clinical Committee will meet bimonthly on the 3rd Thursday of the month from 7:30 am to 9:00 am. The Clinical Committee will meet in January, March, May, July, September and November. The meeting locations will be rotated between Physicians Medical Group and the Santa Cruz Medical Foundation. In addition the Chair as needed may call special meetings. The Chair and Co-chair of the Clinical Committee will meet monthly with the Project Director to plan the next meeting and to respond to clinical questions as CCCN staff. The Agenda and supporting materials will be sent electronically to the Clinical Committee members in advance of the meeting.

 

Project Communications

Clinical Committee members will work to promote understanding between the clinical and quality perspectives on health information technology and the technical, financial and policy points of view. Within the CCCN Committee structure, the work of the Clinical Committee will be integrated with that of the Steering and IT Committees.
