Background
The mission of the Community Chronic Care Network is to ensure that all people with chronic conditions in Santa Cruz County receive consistent and continually improving medical support by employing modern electronic tools and sharing clinical practice innovations among our diverse providers of care.
The Community Chronic Care Network has received grant funding from the Agency for Healthcare Research and Quality (AHRQ) for the period October, 2004 to September, 2007 to implement an electronic registry tool for diabetes care in support of this mission. The Pajaro Valley Community Health Trust (Trust) is the recipient of the grant funds and is responsible for financial stewardship of the Project. The Health Improvement Partnership is the legal entity for the diabetes registry. Participating agencies and providers are contributing matching services, space, equipment, supplies and software to the Project.
Purpose
The purpose of the Steering Committee is to ensure maximum participation in the development of electronic tools and sharing of clinical practices to improve outcomes for people with chronic conditions. This includes guaranteeing each organization’s individual data and practice protocols are respected and full compliance with all applicable regulations for privacy and security of personal health information. In addition, the Steering Committee will provide a forum for rapid discussion and resolution of project issues and develop strategies for continuing these efforts beyond the AHRQ grant funding. The Steering Committee, in conjunction with the Trust, will provide policy oversight for the Project.
Membership
The Steering Committee includes senior leadership from the organizations participating as partners in
the Project. If the designated representative is not able to attend a Steering Committee meeting, the
organization will designate an alternate to participate as a voting member for that meeting.

Chair
A Chair and Vice Chair will be selected at the Fall meeting of the Steering Committee. The Chair will review and approve the Agenda in advance of the meeting and will conduct the meeting. The Vice Chair will lead the meeting in the absence of the Chair.
Meeting Structure
The Steering Committee will meet bimonthly on the 3rd Friday of the month from 9:30 am to 11:00 am. The Steering Committee will meet in February, April, June, August, October and December. In addition the Chair may call special meetings as needed. Meeting locations will be rotated among the partner organizations.
